The failure blew out the clay nozzle cone, which I assume is what blew a hole through the deflection plate on the pad. Both casings remained in the rocket, and upon recovery, the casing was the only thing left where you could see all the way through the casing; the clay nozzle was gone. 2 of 2 of the same package failed.
