Burn-through of forward closure/ejection charge cup. Liner burned through between top grain and adjoining grain, front seal disk hole enlarged significantly (it's now a ring). Delay grain, delay grain spacer, insulator, and o-ring all vaporized. Frame-by-frame video analysis shows smoke coming out the top end of the booster while it was climbing the rail, so it was happening quickly. I have several more pics
