Burned through reload case insulator right where it meets the ejection module. The O rings were both intact and did not appear to have failed

I can send a picture if you need it. The flight was successful it just caused a bulging in the motor casing (using a 6xl with xl spacer) and some burning of the motor mount. Thanks for your time and help.
