There was incomplete and inconsistent combustion of the propellant.

Comments
The motor made a pop when ignited, then seemed to go out, like an extremely severe chuff. It relit later in flight for a split second, then smoldered for the rest of the flight, while the electronics deployed the parachute successfully. On examination, the entire motor casing was bulging out, except for the threaded ends for the closures. Photos of the flight showed that at least some propellant was ejected. The apogee reported by the altimeter was about a third of what the same rocket had achieved on the same motor a few months prior.
