A manufacturing defect with insufficient clay in the nozzle caused the throat diameter to be larger than designed, preventing the pressure to achieve that necessary to ignite the propellant. In the photo, the one on the right is from a more recent lot, and has the proper throat diameter. A more energetic ignitor may be necessary.
