4.5 sec burn turned to a 30 sec. Burn which never lifted the rocket off the pad. I've flown it before with identical set-up and openrocket sim showed 4,200 ft.

To be fair, I suspect the ignitor wasn't fully inserted, essentially turning it into an end-burner. Motor hardware was not damaged at all.
